Dream High In Logansport & Winamac We continue to be amazed at the wonderful outpouring of support from the citizens of these communities. Following the two Dream High Breakfasts, we once again find that your generosity exceeds our expections. We know of individuals, companies and couples that are still considering making a donation or adding the Foundation to their estate planning. As this newsletter goes to print (just two business days following the Winamac event), the total we received this year to provide support to persons with developmental disabilities exceeds $83,000. We are amazed. We are grateful. We are blessed. We will be good stewards of your contributions. We Are Providing Support The Foundation is pleased to announce that support is being provided to individuals on the waiting list. Several individuals applied for support. The Foundation was able to say “yes” to a few and will consider additional applications as they are received. The first three individuals to receive support cover a broad span of agency services. One young man is now being served by Family Support Services. For several hours each month, staff members will help him learn how to become more independent in the community. An older gentleman is receiving several hours of support each month in the home in which he lives with two others. He needs to learn more about handling his medical issues, including such things as personal care, diet and exercise. Yet another man is working at the Winamac Work Services program. For several days each week he will have a job that provides income and the chance to socialize with others. All of these individuals have been on a waiting list. We are proud to be able to provide the support that they want and need. As this newsletter is in process of being printed we are considering additional applications for funding. We hope to be serving several more individuals before the end of 2007.
You May Still Apply For Support If you are one of the nearly 200 people on a waiting list for services in Cass or Pulaski County, you may still apply for bridge funding - funding for support at Peak Community Services from now until the time you come to the top of that list. These are the steps to be taken to become eligible for consideration for support. 1. Complete and sign application information to the best of your ability. 2. Provide proof of being on a waiting list for waiver services for persons with developmental disabilities. 3. Provide up to one letter-sized (handwritten or typed) page of information that explains the circumstances surrounding the need for support. 4. Identify which support services you would like to receive, and how much. The Peak Community Services Foundation will provide funds for support services to men, women and children who are on a waiting list for waiver services. Recipients must agree that once they come to the top of the list and are offered government funds, they will report this change in status to the Peak Community Services Foundation. Assuming funds are available and services are still offered by Peak Community Services, the Foundation will maintain funding for each applicant chosen until government funding begins. Please contact Kathi Thompson to receive an application. |
